1. Criteria for YPT

The Young Person's Team take case responsibility for all those young people who are:

  • Aged 14 years or over;
  • Are Looked After;
  • Have a Permanence Plan, which is appropriately agreed and placement identified.

In order to ensure the transfer process is undertaken appropriately the following is required as a minimum:

  • Case discussion has taken place Children's Social Care Manager (CSCM) to CSCM , with an agreed date for case responsibility to transfer;
  • Transfer form (ICS) has been fully completed;
  • ICS records are complete and up to date (see section on ICS);
  • Documents identified in the attached checklist are completed and identified;
  • The young person, family and carers have been advised of the intended transfer of case responsibility and their comments, issues are recorded in the transfer form;
  • Planning meeting held to confirm transfer arrangements and review actions / care plan;
  • Arrangements are made for a joint visit / meeting to handover case responsibility.


2. Rotation of Transfer Arrangements

It is proposed that the Young Person's Team will accept transfer of children who meet the above criteria on a rotating basis between the area social work teams.  Where a team do not have anyone fitting the specific criteria, the next team will be able to identify a young person.  The team who have missed will have to wait for their turn to then transfer another young person.

The CSCM for the Young Person Team will advise the respective team managers of when the YPT is able to consider transfer, and will make contact with the manager of the specific manager. A transfer meeting will be arranged to discuss the specific issues relating to the young person and to plan the transfer appropriately.  

It is acknowledged that there may be occasions when urgent consideration is required regarding the transfer of a young person to the YPT. This process is designed to consider urgent requests, but to maintain a fair and equitable transfer arrangement to the YPT. 

Where an area CSCM has an urgent request, this must be made, manager to manager in the first instance. The YPT CSCM would then review the capacity in the team and negotiate how they could offer support. This may be in the form of co-working in the first instance, or could involve full transfer of case responsibility. 

NB in the event of an emergency, this does not preclude the need for the following criteria to be met, however negotiation may take place on how quickly this will be completed and by whom. 

  • Case discussion has taken place team manager to team manager, with an agreed date for case responsibility to transfer;
  • Transfer form (ICS) has been fully completed;
  • ICS records are complete and up to date;
  • Documents identified in the attached checklist are completed and identified;
  • Arrangements are made for a joint visit / meeting to handover case responsibility.

Where there is a disagreement between CSCM the matter should be referred to the respective Managers, in order to resolve the disagreement as appropriately as possible.

Where a team have transferred a young person to the YPT outside of their rotating turn, they will not be considered to transfer another young person on their next slot, and will effectively miss a turn.


3. Cohort of Young People  

All Managers will ensure that they maintain an overview of all young people aged 14 years or approaching this age. This information is available and updated monthly and available from the Service Manager.


4. ICS

When the team manager has completed all aspects of the transfer process, they will alert the receiving manager to the request for transfer.

It is the receiving Team Manager who must confirm that this process has been completed and ensure that the Team and Allocated case worker role has been reassigned. Otherwise the case remains the responsibility of the original team. This applies in cases where case responsibility is being handed over form one social work team to another. 

When the manager of the receiving team enters the new allocated worker into the system it will automatically end the previous allocated workers involvement.

Any dispute as to when or if a case should be transferred needs to be addressed at service manager level.
